How to Join

Becoming a member of St. Vincent IPA is easy. First, look in your health plan directory for St. Vincent IPA. Once you have located your doctor, note the doctor's name, location and provider number and fill out the Health Plan Enrollment forms accordingly. You may also switch to a St. Vincent IPA doctor by contacting your health plan and telling them that you would like to change to a St. Vincent IPA provider.
