Comprehensive services for our Homebound Program include:
- Visits in the patient’s home or facility of residence
- Comprehensive physical exams, treatment plans, medication review
- Blood draws, lab work, diagnostic testing, specialist referrals
- Coordination of necessary home health and DME services
- On call access and telephonic support for the patient 24/7
- Family and caregiver education on the patient’s care
- Coordination with the patient’s health plan, social workers, and/or state and county programs for services for which the patient may be qualified
- Complete progress reports regularly provided to the patient’s PCP

- Unlimited follow-up visits to patient as needed to ensure their health and well being
Criteria
- Because of illness or injury, need the aid of a supportive device such as, a cane, crutches, wheelchairs and walkers, the use of special transportation, or the assistance of another person to leave their place of residence or having a condition such that leaving his or her home is medically contraindicated
- There must exist a normal inability to leave a home and leaving home must require a considerable and taxing effort
