We completely understand that your health is one of the most important things in your life. Becoming a member of St. Vincent IPA is easy, just follow these steps:
- Look in your health plan directory for St. Vincent IPA.
- Once you have located your doctor and note the the doctor’s name, location and provider number and fill out the Health Plan Enrollment forms accordingly. You may also switch to a St. Vincent IPA doctor by contacting your health plan and telling them that you would like to change to a St. Vincent IPA provider.
