St. Vincent IPA is an Independent Practice Association (IPA) comprised of more than 220 primary care physicians and more than 450 medical specialists in the greater Los Angeles area. Our physicians and staff have provided quality care to tens of thousands of patients for over 20 years.

Ease of Usage

To streamline the referral process for rapid referral turnaround, St. Vincent IPA utilizes the interactive program Cerecons. This allows for easy and secure referral submission and status tracking for our providers.

Helpful Physician Services

St. Vincent IPA features two services and support programs: Diabetes Program and Homebound Program. Both programs are designed to support our providers’ practice and provide patients with additional one-on-one care and support. Our Diabetic Nurse Educators, supplements provider efforts by providing education and counsel, routine exams and monitoring, and review of medications for patients, as well as providing regular progress reports to the patient’s PCP.

Our Homebound Program leaders, Nurse Practioners help patients who are not physically able to seek routine care from their PCP. This support, provided to complex homebound patients, reduces hospital admissions and re-admissions through comprehensive exams, treatment plans, medication review, lab and blood work, specialist referrals, and additional coordination of care including regular progress reports provided to the patient’s PCP.
